# System Email Templates: Form Emails

## **Form Submission Notification**

### Email Subject

Form submission from *|form.name|*

### Description

This email is sent to the specified email addresses when:

> A learner submits a post-course evaluation

AND

> The [course's **Evaluation Type** setting is set to *Internal*](https://support.smarteru.com/docs/course-post-course-evaluations#evaluation-type)

AND

> The [form's **Send Form Submission Notification** setting](https://support.smarteru.com/docs/editing-a-forms-settings#send-form-submission-notifications) is checked
> 
> OR
> 
> The [course's **Send Form Submission Notification** setting](https://support.smarteru.com/docs/course-post-course-evaluations#send-form-submission-notification) is checked

- [Edit the account's](/v1/docs/editing-your-account-settings) [Email Template Options settings](/v1/docs/account-email-template-options). [Expand the Form accordion](https://support.smarteru.com/docs/account-email-template-options#form-email-types) and select the email template for the [Form Submission Notification email type](https://support.smarteru.com/docs/account-email-template-options#form-submission-notification).
